CLINICAL INTERN....SEM BR DH NK BUKAK BACK ON TRACK...

sibuk2 hal nk bukak sem ngn interview nih mari share2 sket pengalaman2 lepas...utk panduan di masa akan dtg.....hehehe~

One of things that struck me when started my clinical intern at IPR ( institut perubatan respiratori) HKL....is that almost every doctor I met emphasized on answering questions in a systematic manner.

waahhhh ckp english sket esok ader interview penting..!!! prepare2 lamer aku tk ckp english belit2 lidah nnty jd kelantanglish.....hahaha~ sambung2....I remember I was once asked to name the symptoms of lung cancer by WHO...????? DR GOON...woot2...si doctor yg slalu pakai sluar singkat...but he was superb...!!!. I recited the symptoms one by one: cough, hemoptysis, weight loss... ( aku dgn yakin bg jawapan smbil dibantu oleh kawan2 yg menggomel2 di tepi telinga...) BUT..!!!! he was not impressed at all....!!!! urghhhh~ yakin jer aku jawap semua soklan....

then, he told me that i should begin my answer by "Lung cancers can present asymptomatically or with local, systemic, metastatic or paraneoplastic symptoms," before describing the symptoms under each subheading ( wahh aku budget bleyh main hambur jer..ruper2nyer semua tk semudah disangka...!! eyh babe..!! U R DEALING WITH PEOPLES LIFE.....!!!!!!In fact, the symptoms of many cancers can be organized into this way....yeahhh agreed...!! btol jugakk....BACK TO ENGLISH PLEASE....

eherm2.....I think this is a difficult skill to learn and I have yet to master it. But if you start to organize your knowledge rather than remembering the long list of differential diagnosis and investigations, you will find the huge amount of information much easier to digest..(btoll tk tipoooOOoo....tipoooOO doser masuk api neraka ).... On a side note, work on n brush up your presentation skills ( dat was our major problems...). It is an often ignored aspect but it is as important as your medical knowledge...(dats wat Dr Goon told us lahhh )





tadaaaa~ 18y.o malay girl presented with hemoptysis for 2 month associated with fever and l.o.w + l.o.a bla2..pnjng...~~

Dr Reza ( my favorite doctor from US...) always advice study smart not hard...!!! avoid being hardworking but LAZY...!!! he always shared with us his ideas on effective studying told us to avoid becoming "mentally lazy". ( maksudnyerr.....jgn pulun lebeyh ngadap buku smpy ko leteh tk leyh pk aper2... bacer kt bwh aku nk start speaking blk...hehe~)

This is because we might sit in the library for the whole day, reading the material but not critically thinking and processing the information....( btol kdg2 berlagak jer lebeyh buku timbun2 padahal tk masuk ponnn)

I think a starting point to solve this problem is to ask ourselves: "Will remembering this piece of information help my patient?" If not, it is not worth spending a lot of time on it. I also find it quite rewarding to always think about the complications of a treatment....yiihaaaaa~ yiibaaa2...

An example of this is that I sometimes fall into the trap of suggesting "routine" investigations for a particular clinical problem without asking myself whether I really need them. And I have heard this question at least ten times from various doctors directed against me or my classmates: "Why do you want to request this investigation if it does not alter your management plan?"

If you develop a habit for justifying your investigations, it will reduce many unnecessary investigations for your patient in the future.... dh2 bye2...nk oit to uptowmnnnnnnnn.... smpat lagik ko poya2 esokkk ader interbiuuuuuu~ ader aku kisahhh..?? janjii heppi...hehe mwax xoxo...!!!!
